If you are uninsured, LRMC offers programs that allow uninsured individuals and families to receive services for a fee that is based on household size and income. Our eligibility staff can determine if you or your family qualifies for Medicaid, CHIP or other local, state and federal programs. So that we can best assist you, we ask that you bring the following information to your first appointment:    Proof of income for everyone in household. This can be recent pay stubs or tax return statements.    Confirmation letter for any other income (SNAP, SSI, disability, VA benefits, etc.)    Proof of residency. This can be a recent utility bill, lease or county tax receipt.    Photo ID card    Social Security card    Any evidence of insurance coverage.
